Who can submit a report?

Event reporting in 4HSE is open to users authorized by the organization, and can also be submitted from the mobile App directly in the field. Notification recipients are determined by the configured roles and assignments.

Can a near miss be reported anonymously?

Yes. 4HSE allows near misses (incidents that could have caused harm but didn't) to be reported anonymously. Being able to report without identifying yourself lowers the resistance to communicating minor events, which are the ones most likely to go unreported.

How is a corrective action linked to a reported event?

You can link any event recorded in 4HSE to one or more existing actions from the safety plan as a corrective action. Once linked, the action follows its own already-defined activities, timeline, and responsibilities.

What evidence can be attached to a report?

Reports in 4HSE accept documentary and photographic evidence, surveys, and attachments in various formats. From the mobile App, you can attach photos and videos captured at the moment the event is detected.
